Our Healthy Kids Clinic will have influenza (flu) vaccinations available to students later this month. A flu shot consent form was sent home with your student at the beginning of the school year. If you have already completed and returned a flu shot consent form, no further action is needed.

If you are unsure if you completed a form or have any questions, please contact your school nurse. 

If you would like your child to receive a flu vaccine and have not completed the consent form it’s not too late. Please complete this flu consent form and return it to your school nurse. Find contact information for your school nurse here.

Flu shots will be administered on the following dates:

Monday, September 26, 2022
2-12 grade students at Bardstown High, Bardstown Middle, and Bardstown Elementary.

Tuesday, September 27, 2022
Preschool, kindergarten and first grade students at Bardstown Early Childhood and Bardstown Primary.
