Bardstown City Schools is partnering with Healthy Kids Clinic to offer sports physicals for all student-athletes on Monday, June 17  from 8 AM to 3 PM. Physicals will take place in the Health Services Office (nurses office) at the Bardstown Polytechnic Center.

No appointment is necessary, and no copay is required. However, insurance information is needed, as your provider will be billed for the physical.

Before arriving, be sure to download and complete the sports physical form, including a parent or guardian signature. Forms are available at:
